If you have tested positive for COVID-19 be sure to answer the Covid-19 Questionnaire and complete the COVID-19 Return to Play Form.  The link to the form is in the questionnaire as well as clicking the above Covid-19 Information and Reporting tab.

Please abide by this timeline, late registrations do not guarantee the opportunity to participate.
Make sure your physical is on file with the nurse OR upload it with your registration OR email to sskutnik@westportps.org OR cboyrer@westportps.org.

Registration requirements:

  • If this is the first time registering during this school year, a completed registration will result in receiving 5 emails indicating you have agreed to the Positive Covid Questionnaire, Code of Conduct, Concussion Education Agreement, Sudden Cardiac Arrest Agreement, Emergency Information Form and Mandatory Covid Waiver Agreement.
  • If you registered in the fall, you will receive 3 emails -  Positive Covid Questionnaire, Emergency Information Form and Mandatory Covid Waiver Agreement.  
  • If you registered in the winter,  you will receive 2 emails -  Positive Covid Questionnaire and Emergency Information Form.
  • Be sure to include parent and student email addresses in your registration as teams are using this information to communicate with teams.

 Physical Examination Requirements:

  • In consultation with the Connecticut State Medical Society, Sports Medicine Committee, the standard of physicals being valid for a 15-month period prior to the start of the season will remain.  If you do not have a physical you will not be permitted to participate.      
  • Completed physical examination forms must be on file in the School Health Office, attached to your registration or emailed to  .
