First time registering? 

• Please provide a valid e-mail address to receive a temporary password for your member account. 
• Retrieve the temporary password from your e-mail account.
• Return to the sign in page and enter your e-mail and temporary password to begin.
• Student athlete and parent are required to submit the Concussion Informed Consent Form, the Sudden Cardiac Arrest Form and Code of Conduct.
• Fill out all information on the parent/guardian page.  All information marked with an asterisk is required. Click “submit” to proceed.
• Fill out all information on the Participant page. All information marked with an asterisk is required. Click “submit” to proceed
• The final page is a confirmation page. Please review the summary carefully. Changes can be made by clicking the “change” button”
• Your registration is complete when you receive an email confirmation.  Newly registered athletes will receive 6 emails.  
• Once the nurses have approved your sports physical, you will be added to the roster.
